This GP practice is taking part in an important national project about people at risk of diabetes and diabetes care and treatment in the NHS. The project is called the National Diabetes Audit (NDA). For more information click here.
The practice believes in promoting better health, by advocating a healthy lifestyle, and by helping to prevent disease in the first place, and also by reducing the risk of complications in pre-existing disease.
Here are ways in which we can help:
We also run the following Clinics:
Travel Clinic. The practice provides information about medical aspects of foreign travel to registered patients and will vaccinate according to the latest advice. We are a registered Yellow Fever Centre. You do not have to be registered to receive this vaccine but patients must contact their own surgery for all other vaccines.
If you require any vaccinations relating to foreign travel you need to fill out a travel assessment form (see below) and then the practice nurse will contact you to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.
There is further information about countries and vaccinations required on the link below:
It is important to fill out and return the travel assessment form as soon as possible - at least 8 weeks before you travel - as an appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
Travel Health Questionnaire
To help us offer the appropriate advice, please print out the following Travel Pack, and complete the Travel Risk Assessment form (page 3 and 4). Please then return the whole pack to the surgery and we will arrange for someone to contact you to arrange an appointment if necessary.
*PLEASE REMEMBER TO PRINT THE WHOLE TRAVEL PACK - NOT JUST PAGES 3 AND 4*
If you are travelling to Europe the EU has published useful information for travellers on the European website.
Some services provided are not covered under our contract with the NHS and therefore attract charges. Examples include the following:
The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.
