On Thursday 26th May many of our clinical staff at the  practice will be attending an important training event arranged by the Clinical Commissioning Group. There will be a reduced number of appointments available on that day. If you require urgent medical assistance please inform the practice reception. The practice will open as usual at 8.30am the following day.

We would like to notify our patients that our reception team will be asking you some brief details regarding your request for an appointment. Please indicate the nature and urgency of your problem so we can help you as efficiently as possible.  This will ensure that you are booked in with the correct healthcare professional and that the quickest route to treatment or advice is taken. This GP practice is taking part in an important national project about people at risk of diabetes and diabetes care and treatment in the NHS. The project is called the National Diabetes Audit (NDA). For more information click here.


 

 

Vanguard

Recently North East Hampshire and Farnham was chosen as one of a handful of sites nationwide where local clinicians and practitioners will work with communities to create and operate new, joined-up health and care services.

 

Our Vanguard programme has an overall aim of keeping people “happy, health and living at home” wherever possible. It will see local people supported to improve their own health and wellbeing and when they are ill or needing support, they will receive the best possible care.

 

The programme will focus on:

  • prevention and self-care - helping people to manage their own health condition, empowering them to make choices about their care and ensuring the right services are available to all our communities;
  • integrated teams of specialist health and social care professionals - establishing teams of community nurses, occupational therapists, physiotherapists, social workers, a psychiatric nurse and a psychiatrist, a pharmacist, a specialist in care of the elderly, GPs, the voluntary sector, and specialists in palliative care and domiciliary care. These teams will be located in Farnham, Aldershot, Farnborough, Fleet and Yateley;
  • enhanced community care services - for people in their own homes, in GP surgeries and local, community hospitals;
  • access into and out of specialist inpatient care - in community hospitals (such as in Farnham and Fleet) as well as Frimley Park Hospital.

The Vanguard programme is a partnership between NHS North East Hampshire and Farnham Clinical Commissioning Group, Frimley Health NHS Foundation Trust, Southern Health NHS Foundation Trust, Surrey and Borders Partnership NHS Foundation Trust, Virgin Care, South East Coast Ambulance NHS Foundation Trust, North Hampshire Urgent Care and Hampshire and Surrey County Councils and all GP surgeries of the NE Hampshire and Farnham area.

 

We are also working with you, the patient, the public and voluntary organisations to help shape, develop and continually improve the way services are provided.
